Florida Care Licences
Standard, LNS and ECC: what each assisted living licence allows.
State regulation and the AHCA
In Florida, the Agency for Health Care Administration (AHCA) oversees Assisted Living Facilities (ALFs) and their distinct specialty licences.
The difference between them comes down to the level of medical care and nursing assistance a facility can provide to help residents “age in place” without transferring to a nursing home.
The three licences
Standard licence
A Standard ALF licence allows a facility to provide basic housing, meals and personal care, such as help with activities of daily living (ADLs) and medication management.
Limited Nursing Services (LNS)
A specialty add-on that allows an ALF to provide minor, part-time nursing services that go beyond basic personal care, under a doctor’s order, without providing 24-hour nursing supervision.
- Allowed services: routine nursing tasks such as catheter changes, wound care, urine testing and routine maintenance of casts or braces.
- Restriction: the resident must generally still meet standard ALF admission criteria and cannot require 24-hour nursing supervision.
Extended Congregate Care (ECC)
The highest level of care an ALF can offer. It essentially allows the facility to provide any nursing service within a nurse’s scope of practice, so long as it does not require continuous 24-hour medical monitoring.
- Allowed services: total physical assistance with ADLs such as bathing, dressing and toileting, medication administration, and care for chronic conditions or stage 2 pressure ulcers.
- Eligibility: a facility must generally have held a standard ALF licence for at least two years without serious state citations, or obtain a provisional ECC licence.
LNS and ECC at a glance
| Feature | Limited Nursing Services (LNS) | Extended Congregate Care (ECC) |
|---|---|---|
| Primary goal | Provide specific, medically necessary nursing tasks. | “Aging in place”, even with advanced physical or mental decline. |
| Personal care | Basic assistance; must still meet standard ALF admission criteria. | Total assistance and supervision with all ADLs. |
| Nursing tasks | Routine tasks ordered by a doctor, such as wound care and catheters. | Full scope of nursing permitted in an ALF, including total medication administration. |
| Admission | Can only admit residents who meet strict, standard ALF criteria. | Acts as an exemption: residents who wouldn’t ordinarily qualify for an ALF can be admitted or continue living there. |
| 24-hour care | Prohibited. | Prohibited: care must not rise to the level of a skilled nursing facility. |
Standard versus LNS
Care and services
- Standard: strictly limited to assistance with daily living (dressing, bathing, eating) and medication administration. It cannot handle complex medical tasks or daily skilled nursing.
- LNS: allows intermittent nursing services, including caring for stage 2 pressure sores, managing established in-dwelling catheters, applying routine (non-packing) dressings, passive range-of-motion exercises, testing urine and administering eye and ear irrigations.
Admission and retention
- Standard: residents must be able to perform ADLs with help, be capable of self-administering medication or receiving assistance, and not require 24-hour nursing or be bedridden.
- LNS: residents must still meet all standard admission criteria. However, if a resident’s medical condition changes, they are allowed to stay in the facility while using the extended LNS care.
Staffing
- Standard: does not require a licensed nurse on staff or contract. Personal care can be given by trained caregivers or medication technicians.
- LNS: the facility must employ or contract with an active Florida-licensed nurse (RN, LPN or APRN) to perform these skilled tasks or directly supervise them.
How a facility obtains an LNS licence
To add an LNS endorsement, the AHCA requires that a facility:
- Already holds a valid Standard ALF licence.
- Has not been subject to any adverse administrative sanctions affecting resident health, safety and welfare for the previous two years, or since initial licensure.
- Submits a formal application and amends its service plan with the AHCA.
